Hold Descriptions

Original I9 Documentation:  The student employee is required by Federal Law to provide original and unexpired documentation to your employers to show your identity and authorization to work in the U.S. For a list of acceptable I9 documents, click here.  You can find more information and list of form I9 acceptable documents in illustration format here to further assist you in determining what to physically bring into Student Employment, SSB 2100, anytime Monday-Friday, between 9am-4pm.

OIS Clearance: If you are an International Student (F1), there are additional steps required for us to complete on our end and cross check with the Office of International Services office before you obtain work release and your SSN letter of support request.  There is no additional action required on your end.  So please be patient, thanks.

Criminal Background Check and/or Sanction Review: Student employee electronic review, signature to consent, authorize and release information related to their criminal background and/or sanction.

Physical: The position title you are attaining or your employing unit requires a health physical evaluation.  A copy of your physical clearance is still needed.  Please remit to studentjobs@uic.edu.  Include your full name and UIN within the body of the email.

Certification: The position title you are attaining requires for you to have a valid and unexpired certification.  A copy of your valid and unexpired certification(s) is still needed. Please remit to studentjobs@uic.edu.  Include your full name and UIN within the body of the email.

Nepotism Form: Upon UI New Hire completion, you disclosed that you have relatives (related through blood or marriage) who work for the University of Illinois.  The Nepotism - Proposal to Hire Instructions  (form under General/Employment) was provided for you to complete and return in its entirety.  It’s the student employee’s responsibility to gather all the information.  Please do not leave any boxes blank.  We will not accept incomplete forms.  Here is more information related to the policy: UIC HR Policy #313.

UI New Hire: Your logon credentials were created and sent to you through a system generated email.  You are required to complete all the UI New Hire forms: My Profile (EIF), Loan Default Status, Withholding Allowance (W4), Direct Deposit, Ethics Orientation, Code of Conduct, W2/1042S/1095C Consent Form, Drug Free Workplace, Title IX Training, ANCRA, I-Card Terms, Insurance Marketplace, and Identity Verification.
